Migrate a voice character to v3.0

Migrate a v1.0 / v2.0 voice character to v3.0. Flow and error semantics are identical to migrating to v2.0,
except the "needs migration" check: when version is already v3.0, VOICE_MIGRATE_NOT_NEEDED is returned (modules/voiceFactory.js:1006).

Sub-code semantics of failure reasons (important)#

VOICE_MIGRATE_NOT_ALLOWED is a parent code shared by two entirely different reasons;
sub-codes only rewrite message and do not enter the HTTP body:

The client can only distinguish by message, not by code.
A v4.0 voice cannot be migrated through this endpoint, nor can it be a migration target — it has no upstream character to rebuild, and cross-family migration needs a separate pipeline (modules/voiceFactory.js:999-1001).
Legacy alias: /api/tts/voice/{id}/migrateV3 — still supported for backward compatibility; use the primary path for new integrations.
Error responses take the shape { status, code, message, requestId? }. code is always the parent code — sub codes only change message and never appear in the body. requestId is present only on reportable-tier errors, so use the X-Vocu-App-Request-Id response header when troubleshooting (it is written unconditionally on every response).
